Join me in this workshop where we will fill out the Five Wishes document and discuss the importance for planning ahead.
Five Wishes was the “first advance directive to address personal, emotional, and spiritual issues in addition to meeting medical and legal criteria” and is America’s most frequently used living will. It is legal in most states, including Maryland and Virginia as well as DC.
The document covers personal, spiritual, and emotional wishes as well as kinds of medical treatments you may or may not want. The document also asks you to name a Health Care Agent (in case you are no longer able to make your own healthcare decisions).
This document only takes effect if you are unable to make your own decisions or speak for yourself.
Changes to our health by illness or accident can happen to any of us. The time to plan is now. The Fives Wishes ensure that your wishes have been documented. Your loved ones do not have to guess as to what you would want, and they will know they are honoring your wishes.
We will discuss how to share your Five Wishes with your physician and your family/friends. Sharing your Five Wishes with others is KEY to making sure people know what you want (or don’t want).
You will be given your own Five Wishes document to write in (and sign and take home). Light snacks and drinks will be provided. Rhizome is near the Takoma Park Metro station. There is a ramp at the back door. ***The only bathroom is up one flight of stairs.***
